Note: ICI reserves the right to change incorrect registration types chosen by registrants up to 30 days after the event. If the registrant has an outstanding balance, ICI will notify the individual within the 30-day period to request payment.

Note: Payment must accompany registration form. Registrations received without payment information will not be processed. A notice of confirmation will be emailed within five (5) business days for online registrations, and within 10 business days for registrations with check payment.
Cancellations received by Friday, October 25, are subject to a $75 administrative fee. After Wednesday, October 25, we regret that no registration fees can be refunded. Notice of cancellation must be received in writing.

Please make your hotel reservations by calling the hotel directly (415-392-3434 or 800-662-4455) by Thursday, October 17, 2012. Identify yourself as an ICI conference attendee to receive the king classic rate of $265 per night (not including tax). Reservations are guaranteed with one night’s deposit using a major credit card. Please note that the hotel block may sell out before the above cutoff date, so make your reservation as soon as possible. The contracted rate is based upon room availability.
HOTEL CANCELLATION POLICY
The hotel must receive cancellations 72 hours prior to date of scheduled arrival to avoid penalty. The hotel block is intended for the use of registered conference attendees. When cancelling, please note and retain the cancellation number quoted by the reservation agent.
COMMITTEE MEETING DRESS
Business casual dress is customary for all committee meeting events.
